Coders play a vital role in connecting healthcare providers, patients, and insurance companies. Specifically, the duties of the position include: Your Responsibilities: Review and analyze medical records to assign accurate ICD-10-CM diagnosis and ICD-10-PCS (if applicable) procedure codes. Ensure coding accuracy and compliance with CMS, Medicare, Medicaid, and state-specific requirements. Apply PDPM coding guidelines to optimize reimbursement in skilled nursing facilities (SNFs). Work closely with clinical staff, therapists, and providers to clarify and improve documentation for accurate coding. Conduct routine and random audits of facility coding practices to ensure accuracy and compliance. Identify coding errors, documentation deficiencies, and trends that may impact reimbursement or compliance. Provide audit reports with findings, recommendations, and corrective action plans. Assist in educating facility staff on proper documentation and coding practices to enhance compliance. Track and report coding trends, common errors, and facility-specific coding concerns to leadership. Partner with MDS coordinators, Business Office Managers (BOMs), and regional teams to ensure accurate diagnosis reporting and compliance. Other duties, responsibilities, and activities may change or be assigned at any time with or without notice. Requirements: CPC (Certified Professional Coder), CCS (Certified Coding Specialist), CCA (Certified Coding Associate), or CRC (Certified Risk Adjustment Coder). Associate’s or Bachelor’s degree in Health Information Management, Medical Coding, or a related field preferred. 3+ years of ICD-10 coding experience in long-term care (LTC), skilled nursing (SNFs), assisted living, or senior living settings. Experience in auditing and quality assurance of coding and documentation. Strong knowledge of PDPM, Medicare, Medicaid, and multi-state regulatory requirements. Familiarity with Electronic Health Record (EHR) systems and coding software.
